Hello, Can I speak with (PT Name), My name is (Agent) and I'm calling from the shipping department at (DME Name) in regards to your orthotics. We have your order ALMOST ready to process and just wanted to verify a couple of pieces of information with you before we ship it out.
First things first we want to make sure that we have the correct address. The address we have on file is (READ Customer Address). Is that correct?

Okay perfect. We have you receiving (List Braces). Is that correct?
(If YES, Continue)
(If NO, Verify what Braces they should be receiving)
Okay awesome. Now the phone number we called you on is (Read Phone). Is this the best phone number to reach you at? Do you have an additional phone number you want to put on file, just in case we have to get a hold of you regarding your order?
Perfect. You are all set. We are going to start processing your order. Once your order is finished processing you should receive your braces in 1 - 2 business days. When you receive your braces there will also be a sheet in the box that will have a survey, if you don't mind taking a few moments to fill that out and send it back with the pre-stamped envelope we would really appreciate it.
